  • RX number (most important)
  • Medication name + strength (e.g., 20 mg)
  • CVS store phone number (specific location)
  • Your legal name + date of birth (exact spelling)
  • Insurance info (BIN/PCN/Group/Member ID if you want Costco to bill your plan)

How to transfer prescriptions from CVS to Costco online

Here’s how to do a CVS to Costco prescription transfer online:

  1. Open Costco’s prescription transfer form and select your Costco pharmacy location.
  2. Enter your patient info (name, DOB, phone) exactly as CVS has it.
  3. Enter the CVS pharmacy info (store phone number and location).

No refills remaining
